Leaving our hospital
It has been our privilege taking care of you and we hope that you will depend on our compassionate hospital team for any future healthcare needs.
Preparing for discharge
It is our goal to help you have a smooth transition when you are being discharged from our hospital. We begin planning for your discharge upon admission and will discuss the process throughout your hospital stay. Discharge occurs every day at 11:00am, and is directed by physician written order.
Determining follow-up care
The healthcare professionals caring for you will work with you and your chosen support system to determine the care you may need after you leave our hospital. We will also help determine what resources are available to meet these needs. Your dedicated team may include:
- Case managers
- Dietitians
- Laboratory technicians
- Nurses
- Occupational therapists
- Pharmacists
- Physical therapists
- Radiologists
- Respiratory care practitioners
- Social workers
- Speech pathologists
A nurse case manager will be available to assist with arranging outpatient services and addressing any concerns you may have about discharge. They will also be in communication with your insurance company to keep them informed of the care you receive while hospitalized and what services you will need after discharge.
Transportation
Please plan in advance for your transportation at discharge. When you are discharged, transportation arrangements should already be in place.
Often individuals providing transportation at discharge will request a time of their convenience. Unfortunately, it is difficult for us to honor this request because other people are likely waiting for a bed and care on the unit.
